The Behavioural Support Program has been implemented at Ivan Franko Home since 2018, in collaboration with the Toronto Central LHIN.
Our Behavioural Support Lead serves as an in-house resource person assisting staff in identifying possible triggers and/or contributing factors of responsive behaviours that may be associated with dementia, mental health, substance use and/or other neurological conditions.
The program focuses on supporting and managing residents with responsive behaviours, providing ongoing person-centered care for residents with all types of behavioural responses that meet their individual physical, intellectual, emotional, psychosocial, functional abilities and spiritual needs.
The Behavioural Support Lead works in partnership with: Baycrest Outreach Team, Nurse Lead Outreach Team, Psychogeriatric Research Consultant, Geriatric Mental Health Outreach Team, Alzheimer's Society, hospitals, primary care providers, families/POA and other providers and professionals.
